Abstract
Esopahageal achalasia is a disease of unknown etiology characterized by abscence of peristalsis in the body of the esophagus and failure of the inforior esophageal sphincter to relax in response to swallowing.
It was first described in 1674 by Thomas Willis, who treated dysphagia and regurgitation employing dilatation.
In 1926, Rake first demonstrated degeneration and decrease in number of the ganglion cell of Auerbach¢¥s plexus. It is generally agreed that neuromuscular incoordination is the basic abnormality in this disease.
Several types of treatment have been tried, with drugs, diets, psychotheraphy and also with dilatation and operative procedures.
In 1913, Haller employed esophagomyotomy with two longitudinal incision of anterior and potserior wall of the esophagus. This procedure has been modified by others. Now, the modified Heller¢¥s technic of esophagomyotomy is the treatment of choice on achalasia.
In 3rd Army hospital, one case of advanced achalasia was treated with modified transthoracic Heller¢¥s operation to being a successful result.
We are to report this case to the Korean surgical society.
